Social Prescriber

Are you struggling mentally, physically, emotionally or financially and feel completely alone and stuck? Well you just may not be aware of a service through your local GP practice that assists you with finding the supports for these issues.


 

Talk to your GP


While your GP will Diagnose and prescribe medications for illnesses and symptoms, the GP can also refer you to a Social Prescriber that will assist you with societal difficulties and personal wellbeing.


 

What is a Social Prescriber?

Social Prescribing is a community support service through your local Doctors surgery.

Depending on your needs the Social Prescriber will find and allocate appropriate services to assist you.

For example;

  • Weight and fitness concerns - Fitness centres

  • Trauma and emotional pain - Therapy

  • Money worries - Financial advice/ Support

The list goes on. The support is determine on the help you need.
